I, the undersigned parent/legal guardian of the above-named child, give permission for my child to attend and participate in the Ascension Prayer Group. I understand that the event is faith-based in nature and will include group ascended prayer, discussion, and fellowship.

I acknowledge that reasonable supervision will be provided by group leaders, and I release the organizers from any liability for accidents or injuries that may occur, except in cases of gross negligence or willful misconduct.
